Lume Treatment Plan Agreement

At LUME, we strive for transparency regarding our fees. After a thorough examination, we tailor your treatment plan accordingly. While we aim to anticipate all factors, occasionally unforeseen issues may arise beyond our control.

The estimated fee assumes that nothing unexpected is discovered during your treatment and we will make every effort to adhere to the proposed treatment plan. However, clinical situations requiring reassessment, may necessitate adjustments to the treatment plan for optimal results. Any deviations will be promptly discussed with you, including potential additional costs, before proceeding with the related work. In such instances, a revised estimate will be provided as soon as possible.

The provided quotation remains valid for a period of 90 days from the date of receipt.

Cancellation Policy

We understand that unforeseen circumstances may arise, requiring you to reschedule or cancel your dental appointment. In order to provide optimal care to all of our patients and manage our schedule efficiently, we have implemented the following cancellation policy:

Notification Period: For Check-ups and general dentistry, we kindly request that you notify us of any changes to your appointment at least 48 hours in advance.

For appointments booked with a duration exceeding 5 hours, we kindly request a minimum of 1 week’s notice for cancellations or rescheduling.

Cancellations made outside of the designated notification periods outlined above will necessitate a nonrefundable down payment.
